Signing up for FORMED has never been easier!

1. Go to www.formed.org with your web browser
2. Click on Enter Code (on the bottom of the page)
3. Enter Parish Access Code: CRJ4WQ
4. Enter your email and create a password
5. You are in! Each time you log on you will do so with your email address and the password
